Enrollment Requirements

We follow strict guidelines for maintaining immunization and enrollment requirements as mandated by the district and the state.

Health Regulations

State health regulations for students require that each student must have the following items on file at the school they are currently attending:

  1. Immunizations—A current and valid Kentucky Immunization Certificate is due within two weeks after the first day of attendance.

  2. Medical Examinations—Students entering school for the first time must have a Kentucky Preventable Health Care Examination form, which is due within two months after the first day of attendance.

  3. Vision Examiniations—Students entering school for the first time, ages three, four, five or six years old only, must have a Kentucky Vision Examination Form. The form is due no later than January 1 of the first year that the child is enrolled in a public school, public preschool, or Head Start. This examination must be completed by an optometrist or ophthalmologist.

  4. A dental exam is also required for entrance into kindergarten.

Birth Certificate

A certified copy of each student's birth certificate is required to be on file at the school they are attending.

No Smoking

All JCPS locations are tobacco free 24 hours a day, 7 days a week. This includes facilities, public grounds, and vehicles on JCPS property effective Monday, July 1, 2013. This includes all employees, parents, students and visitors on JCPS property, sponsored events, etc.
