• Medication administration

• Promotes health and wellness

• Organizes health clinics for students and staff

• Secures and maintains health records, including physical, immunization, dental and vison exams

If your child has a medical condition requiring medication or treatment, please contact your school nurse and she will provide you with the appropriate paperwork in order for her to care for them at school.

Health Requirements For All Students

• Up to date Immunization Certificate

• Kentucky Preventative Health Care Exam form (Physical)

• Kentucky Vision Examination form (The examination must be completed by an optometrist or ophthalmologist)

• Kentucky Dental Examination Form

All students are required to have all of these forms on file with the school nurse within the FIRST TWO WEEKS OF SCHOOL. Failure to submit these forms will result in your student being excluded from school until they are acquired.
