The Work-Based Learning (WBL) Application is REQUIRED before enrolling and/or participating in a WBL experience. Students can submit online or be returning a completed PDF to school officials.

The WBL Student Candidate Plan must be completed by school officials and finalized prior to the start of a WBL experience. Once completed, all parties (Student, Parent/Guardian, Employer, and School Officials) will review before signing their WBL Agreements. Students can review a blank WBL Plan for information purposes ONLY.

Hazardous Occupations Addendum
The Hazardous Occupations Addendum is embedded in the WBL Plan completed by school officials. This form is for Apprenticeships or Co-ops ONLY . School officials should review the Faculty Guidebook for help determining if form is needed. Employers may be asked to complete the form below by school officials to assist in completing a WBL Student Candidate's WBL Plan.

The Medical Authorization is REQUIRED for students to participate in Work-Based Learning experience(s). Parents/Guardians will complete and submit this form prior to the start of any WBL experience.

The Worksite Survey is a vital tool for school officials to ensure Work-Based Learning students are employed in safe and compliant environments. Schools should use this tool to understand the areas and topics to inquire about during the required initial worksite visit.

The WBL Contact Log (Monitoring Log) is REQUIRED to be completed by school officials for the duration of a WBL Student's experience. The log is used to record contact made with employers and students during in-person site visits and electronic communications.

The Work Hours Verification confirms completed Work-Based Learning experience hours. WBL Students enrolled in Experience-Based Work and Outside Work Experience must provide evidence of 300 cumulative hours. Employers will complete this form if asked to confirm work hours by a student or school official.

The WBL Plan: Change Request is REQUIRED for any changes that need to be made to a current WBL Plan. This template should be completed and submitted to school officials.
The WBL Thank You Email Template should be used by all WBL Students upon completion of placement. This email will be sent to the WBL Employer and includes links to the Employer Evaluation and Work Verification (if applicable).
The Employer Advisory Email Template may be used by school officials to offer a formal invitation to WBL Employers to join their Advisory Board. School officials will determine usage of template.
